Please reschedule the appointment if you or anyone in your household has been recently ill or ill during the past month or you believe that anyone has been exposed to another individual with COVID-19.
Only one adult may accompany the child, no siblings. A face mask, and face shield are required for the adult, and a face mask for the child.
Due to the COVID-19 situation, we are reducing contact and we will need to have all of the screening forms filled out and returned to us before your appointment date or your appointment will be canceled. To receive the needed forms please fill out this google form by clicking the link below:

The Farmington School District is providing Early Childhood Screening for preschoolers. All students entering kindergarten in a Minnesota public school need to have proof of having attended an Early Childhood Screening. All children have the right to receive a FREE Early Childhood Screening starting preferably at age 3. Children can be also screened at age 4 and 5.
The purpose of the screening is to provide parents with general information about their child’s growth and development and to connect families with resources in the community. Children who attend Head Start or Early Childhood Special Education programs may not need to be screened.The Early Childhood Screening appointments are held on select days throughout the year between 9 am-4 pm at Meadowview Community Ed, 6100 195th St W Farmington.
Interpreters are provided if needed. Parents can request interpreters when registering or by calling our office @ 651-460-3200.
