Print it out and submit it to Farmington Community Education 6100 195th St W, Farmington, MN 55044 or 2. Complete it electronically, save it, and email it to chenderson@farmington.k12.mn.us. Please note the screening appointment date in the subject line of the email. 3. Bring it with you to your appointment. Each child will need all of the following: 1. Screening Packet 2. One social/emotional questionnaire. Please complete one of the following based on the age of the child at the time of the screening. 36 months – 42 months of age 42 months and 1 day – 54 months of age Over 54 months of age 3. A copy of your child’s immunization record (this can be in any format). If you are a conscientious objector, please provide a copy of your notarized objection.4. A copy of your child's birth certificate. You can complete the paperwork at your appointment, but please allow extra time to do so. Thank you! Interpreters are provided at no cost to families. Please let us know if you or your child will need an interpreter during the screening. Please note that family members cannot interpret for children.

Early Childhood Family Education (ECFE) is a fun and supportive program designed for parents and their children from birth through third grade. ECFE classes provide a welcoming and relaxed environment for families to connect, learn, and engage in thoughtful discussion with the guidance of a parent educator. Simultaneously, children will participate in developmentally appropriate activities across various domains, including art, literacy, mathematics, science, and gross motor skills..Please remember that all children aged 6 months and older are required to be registered and to pay for the class. Young Rembrandts Grades 1-5 Wizarding Academy Art Day

Beginner 2 (ages 5 and up – 55 mins) Beginner 2 is for gymnasts who have tested out of Beginner 1 class or have been evaluated at this level. This class involves the introduction of basic gymnastics skills on all four apparatus: vault, bars, beam and floor. Tumble Track is also incorporated to teach jumps as well. An example of skills introduced are forward and backward rolls, handstands on floor, glide swings on bars, forward and backward walks and straight jumps on beam. Beginner 2 gymnasts must be 5 and have tested out of Beginner 1. All payments will be charged one week before the classes begin on Wednesday.

Intermediate 1 & 2 (ages 5 and up – 70 mins) This class is for gymnasts who have tested out of Advanced Beginner or Intermediate 1 and have been evaluated at this level. This class also utilizes all four apparatus and trampoline. Gymnasts will continue to perfect the skills introduced to them in Advanced Beginner and Intermediate 1. They will also begin working on round–offs, bridge kick-overs as well as moving their basic floor skills to the low and medium balance beams. All payments will be charged one week before the classes begin on Wednesday.
